Kcentra contains heparin [see Description (11)]. Hypersensitivity reactions including flushing, urticaria, tachycardia, anxiety, angioedema, wheezing, nausea, vomiting, hypotension, tachypnea, dyspnea, pulmonary edema, and bronchospasm have been observed with Kcentra [see Adverse Reactions (6.2)].

What is Kcentra made of?
Kcentra is a purified, heat-treated, nanofiltered, lyophilized, non-activated 4-factor prothrombin complex concentrate (4F-PCC) made from pooled human plasma. Kcentra contains all 4 vitamin–K dependent coagulation factors (II, VII, IX and X), and the antithrombotic Proteins C and S.
Is Kcentra a blood thinner?
Kcentra (prothrombin complex) is a blood coagulation factor replacement product. Kcentra is used to quickly reverse the effects of a blood-thinning medicine (such as warfarin) during a major bleeding episode, or when there is a need for emergency surgery or invasive medical procedure.
Does PCC have heparin?
As PCC products contain heparin, they are contraindicated in patients with heparin-induced thrombocytopenia.

Effects of a single dose are seen in 30 minutes, 3,10 and mean infusion duration is approximately 24 minutes (+ 32 minutes). 10 Redosing with Kcentra® has not been studied and is not recommended.
How long does Kcentra last?
Major bleeding despite INR: Administer vitamin K 5-10 mg IV with Kcentra INR will decrease over ~24- 48 hours (oral) and ~12-14 hours (IV); more frequent INR monitoring and additional vitamin K doses may be necessary.
Is Kcentra FDA approved?

Why is Kcentra contraindicated in DIC?
Don’t use it in a patient with heparin-induced thrombocytopenia (because Kcentra contains heparin). Don’t use it in disseminated intravascular coagulation (DIC) as the addition of coagulation factors will be “adding fuel to the fire.” Instead, treating DIC hinges on addressing the underlying problem.

FEIBA® is current formulary activated prothrombin complex concentrate (aPCC) product. KCentra® is current formulary prothrombin complex concentrate (PCC) product. Kcentra™ contains clotting factors II, VII, IX and X. It contains heparin and use should be avoided in patients where heparin is contraindicated.
Is heparin only IV?

Dose based on actual potency is stated on the vial, which will vary from 20–31 Factor IX units/mL after reconstitution. The actual potency for 500 unit vial ranges from 400–620 units/vial.
Does Kcentra reverse eliquis?
Cofact may be partially beneficial in the reversal of the anticoagulant effect of apixaban. Kcentra currently is the only nonactivated 4F‐PCC available in the United States, and could potentially have some activity against apixaban induced anticoagulation, but has not been systematically evaluated.
